What is Support Coordination?
Support Coordination is facilitating a Participant’s connectivity to various supports. This can be informal, mainstream or those funded by the NDIS. The process includes managing crisis situations, fostering resilience and competence within a Participant’s network, and organising supports from multiple sources.
This process includes:
Participant Intake and Assessment – The first action taken by us will be to assess if Blue Rabbit is the right fit and can facilitate the provision of services for the Participant. A comprehensive needs assessment will then be completed to assess the unique requirements for the Participant.
Initial Coordination – A collaborative approach where the services needed are identified and organised for the implementation of the Participant’s NDIS Plan.
Ongoing Support – This refers to the active management and ongoing adjustment of supports in response to a Participant’s changing needs, managing multiple or complex supports from a variety of providers and helping Participant’s build resilience and navigate through crisis when they arise.
Monitoring and Review – Regular monitoring and review of support services is critical, with three monthly meetings with Participant’s to assess their progress, review their needs and service agreements, and adjust plans as necessary. This is to ensure delivery remains appropriate, relevant and that the services continue to assist the Participant to achieve their individual goals.
Reporting – This includes the provision of progress reports on Participant’s specific goals, outcomes and success, and monitor progress towards anticipated outcomes.

We review NDIS plans and identify areas for improvement and additional supports.
We help prepare for and attend NDIS planning meetings, advocating on your behalf.
We will help you to achieve the appropriate funding needed, to assist you in reaching your goals.
We identify and liaise with the most appropriate supports for you through your funding, and provide ongoing support throughout the term of your plan to address any barriers in the implementation of the plan.
We record progress, provide reporting to each individual and track the plan to ensure funding is being used effectively.
We always ensure that each individual is treated with dignity and respect and has quality of life.
